Lansing Urgent Care performs MHSAA sports physicals at all clinics on a walk-in basis. A parent or guardian must be present during the physical with the student athlete. To save time during registration please complete the health history information section of the MHSAA Form below prior to your visit.
Annual health physicals covered by insurance are not performed at Lansing Urgent Care, please see your primary care physician for completion of these annual physicals.

What is a sports physical?
A sports physical, also called a pre-participation physical examination (PPE), helps determine whether an athlete is healthy enough to safely participate in sports and other physical activities. During the exam, a medical provider reviews the athlete’s medical history, performs a physical examination, and identifies any health concerns that may require additional evaluation before participation.
Who needs a sports physical?
Many middle school, high school, college, and recreational sports programs require a sports physical before athletes can participate in practices or competitions. Requirements vary by school and athletic organization, so it’s important to check with your coach or school before the season begins.
What is included in a sports physical?
A sports physical typically includes a review of medical history, height and weight measurements, blood pressure, vision screening, heart and lung examination, abdominal examination, musculoskeletal evaluation, joint flexibility, strength testing, and assessment for previous injuries or medical conditions that could affect athletic participation.
Can Lansing Urgent Care complete my sports physical forms?
Yes. Lansing Urgent Care performs sports physicals and completes required athletic participation forms during your visit. Please bring any paperwork provided by your school, league, or athletic organization so it can be completed before you leave.
How is a sports physical different from a regular physical?
A routine physical focuses on your overall health, while a sports physical specifically evaluates your ability to safely participate in athletic activities. The exam places additional emphasis on your heart, lungs, joints, muscles, flexibility, previous injuries, and any medical conditions that could affect sports participation.
How long does a sports physical take?
Most sports physicals take approximately 20 to 30 minutes. If additional medical concerns are identified or extra paperwork is required, your visit may take slightly longer.
What should I bring to my sports physical?
Bring your athletic participation forms, a valid photo ID if applicable, insurance information, a list of current medications, glasses or contact lenses if you wear them, and information about previous injuries, surgeries, allergies, or chronic medical conditions. Parents or guardians should accompany minors whenever possible.
Do I need an appointment for a sports physical?
No. Walk-ins are welcome at Lansing Urgent Care, and you can also save your spot online before arriving. We offer convenient sports physicals throughout the year, including during the busy summer and back-to-school sports seasons.
Will I be cleared to play sports during my visit?
In many cases, yes. If your examination is normal, your provider can complete your sports clearance forms during your visit. If additional evaluation is needed because of a medical condition, previous injury, or concerning findings, your provider will explain the next steps before granting clearance.
