What is a return-to-work evaluation?
A return-to-work evaluation determines whether an employee can safely resume regular job duties after an illness, injury, procedure, or period away from work.
When is a return-to-work evaluation required?
An employer may request an evaluation after a work related injury, prolonged illness, surgery, hospitalization, or when an employee has medical restrictions.
What happens during a return-to-work evaluation?
The provider reviews your condition, treatment, recovery, symptoms, job responsibilities, and ability to safely complete required tasks.
What should I bring to the evaluation?
Bring a photo ID, employer forms, job description, previous medical documentation, work restrictions, and any instructions provided by your employer.
Can urgent care provide a return-to-work note?
Yes. When medically appropriate, a provider may issue documentation indicating whether you may return to work and whether temporary restrictions are recommended.
Can a provider recommend light duty?
